Carleton U Arena Rules
Under 12 participant form
Over 12 participants and spectators
NOTE: The below is specific to anyone entering the building and not just those participating in the event - including parents who wish to tie skates or watch the practice (only one spectator per player under 18) and coaching staff/volunteers.
Each team is required to complete the COVID screening form at Carleton U prior to entrance to the facility as well as provide the following:
All individuals that are eligible under Ottawa Public Health's vaccination age guidelines (12 years of age or older as of December 31, 2021) must attest to receiving their first dose vaccine by September 8, 2021, and their second dose no later than October 15, 2021.
All those participating in in-person activities at Carleton University will be required to upload their proof of vaccination through cuScreen by the deadlines referenced above. More information on how to set up a cuScreen account and to provide proof of vaccination will be communicated at a later date. Any false information or misrepresentation on the mandatory attestation form is a violation of university policy that will be taken very seriously.
Any individual who cannot be vaccinated on medical grounds or other protected grounds recognized by the Ontario Human Rights Code can request an exemption. Unvaccinated individuals without an approved permitted exemption will not be able to enter any Athletics Facility.
Vaccination against COVID-19 is the single most effective public heath measure to reduce the spread of COVID-19. Its inclusion among the other public health measures (including physical distancing and capacity limits, and indoor mask use) is essential in the response to the COVID-19 pandemic. We will continue to work closely with public health officials and will continue to adapt as their recommendations evolve.
